• Step 2 – Mail Hair & Saliva Sample Instructions:

  • Hair Sample: ​

    • Gather a few strands of hair (loose hairs are fine or from the back of the head near the neck). Color treated hair is acceptable. Place in a ziplock bag and seal. ​

  • Saliva Sample:

    • Swab the inside of the cheek for about 20 seconds with a q-tip. Place q-tip into a ziplock bag and seal.​

  • Mail samples to PO Box 457, Nashville, IN 47448​

• Step 3 – Schedule Your Telehealth Consultation 14 days from today​​​

  • ​Allowing time for me to receive samples in the mail​

• Step 4 – Receive Your Action Plan

​

• During your Telehealth Consultation we will go over your intake forms and conduct scanning using your samples.

​

• After you Telehealth Consultation I will email an Action Plan of my recommendations and mail out your imprinted patches. Allow 7 days for me to send these out.
